Know Your Physician
Always Event: Always ensure the entire care team and patients and families are able to identify the doctor assigned to hospitalized patient’s care each day in order to improve communication and ability to respond quickly to patient and family questions about their care.
A 2009 study demonstrated that 75% of hospitalized patients in one hospital were unable to identify a single physician assigned to their care. This finding resulted in feedback from both staff and patients that they were unable to effectively communication information between patients and physicians, to seek answers to questions related to care plans and medications, or to quickly provide critical results to the appropriate doctor.
The project team set out with the aim of developing a communication process so that every caregiver involved in caring for a Med/Surg patient knows the Attending Physician and Resident responsible for the patients care.
The solution included 4 parts:
- Definition of different physician’s roles and responsibilities
- Consolidation of several Call Schedules into one mater scheduling tool
- Creation of a process for assigning and sharing the in-charge attending physician
- Development of a new EMR report that pulls the patient census and attending physician daily.
