Safe Patient Mobilization (SPM) Program

Overview: The Safe Patient Mobilization (SPM) program is an initiative designed to always engage interdisciplinary staff, patients and families in fall prevention as a safety imperative. The Sharp Memorial Hospital SPM program implemented a constellation of evidence-based practice fall prevention solutions that gained staff and patient support resulting in a dramatic reduction in patient falls. The validated measures used to assess the effectiveness of the always event fall prevention program include the patient fall rate (nationally benchmarked through the Collaborative Alliance for Nursing Outcomes) and HCAHPS survey question score related to responsiveness of staff through (nationally benchmarked through Centers for Medicare and Medicaid Services and Press Ganey, Inc.). Outcomes to date are a reduction in fall rate by 53% and an incremental increase in HCAPHS score related to responsiveness of staff by 34 percentile points.
